Report Income Change

  • Income Change – one month paycheck stubs, unemployment letter, loss of wage letter, loss of employment, termination, salary change letter

Report Family Composition Change

  • Death certificate – for deceased family member
  • Birth certificate – for adding a new addition
  • Social Security – for adding a new family member
  • Certificate of Blood – for adding a new family member
  • Income – for adding a new family member that is employed or receiving any benefits or wages (job offer, one month check stubs, benefits letter/award)
  • Loss of Income – loss of employment, reduction of wages, termination, or resignation

Report Other Circumstances

  • Other Circumstances – proof of document addressing other issues. This could be a member of the family entering the military, a student moving away to college, a temporary stay (up to 14 days) of a family member.
